If you are referred to us for a consultation or test, you can request an appointment by phoning our rooms or completing the on-line form on our Contacts page.
What to bring to your appointment:
- A letter from your referring doctor
- All relevant X-rays, CT scans, MRI scans (films and reports), and any other previously performed investigations which might be relevant to your consultation
- Your current Medicare Card and Pension, Veterans Affairs or Health Care Cards
- New Patient Details Form with your contact details and medical history (new patients only)
Please confirm your attendance one week before your appointment. If you are unable to attend, please let us know so that an alternative time can be arranged for you and another patient can take your appointment.
Note: If you do not bring the above information to your consultation, it is possible the doctor will not be able to see you on that day and a new appointment will need to be scheduled. If this service is or becomes related to a third party or worker's compensation claim, you will need to inform us of this.

We will collect your personal information for the sole purpose of providing neurological and neurophysiological services. The provision of these services includes the obtaining of results of pathology, radiology and other related tests.
We will only use or disclose your personal information for the purpose of providing health services or where required to by law.
All tests and consultations attract a Medicare rebate unless a worker's compensation or third party insurance claim is pending. Discounts are available for pensioners and health care card holders, and we bulk bill pensioners over 80 years old.
Payment for services is required on the day of attendance. We accept cash, EFT or credit card, and we can submit your account to Medicare for payment. Accounts overdue for 45 days will be sent to an outside agency and commission will be added to the account.
Special arrangements need to be made for third party or worker's compensation accounts - please advise our staff when you make your appointment.
